Enrollment & Eligibility
Eligibility Criteria
Have a documented developmental disability that occurred before the age of 18 and is expected to continue indefinitely. Qualifying disabilities include:
Intellectual disabilities (Full Scale IQ of 70 or below)
Severe forms of autism
Spina bifida cystica or myelomeningocele
Cerebral palsy
Prader-Willi syndrome
Down syndrome
Phelan-McDermid syndrome
Be a resident of Florida, establishing domicile as provided in section 222.17, Florida Statutes.
Be at least 22 years old or have graduated from the public school system with a standard diploma.
Enrollment in Three Hearts ADT Program
Once APD determines eligibility and you are approved for services, you can enroll in our Life Skills Development Level 3 – Adult Day Training program. Our ADT services are designed to support participation in meaningful community routines, including volunteering, job exploration, accessing community resources, and self-advocacy, in settings that are age and culturally appropriate.

At Three Hearts ADT, LLC, we are committed to providing exceptional services to adults with developmental disabilities through our Life Skills Development Level 3 – Adult Day Training program. To participate in our services, individuals must meet specific eligibility criteria established by the Florida Agency for Persons with Disabilities (APD).
Application Process

Obtain and fill out the application for services from the APD. This form can be accessed online or by contacting your local APD office

Submit proof of the developmental disability diagnosis. Acceptable documentation may include medical records, school records, or psychological evaluations. If you do not have this documentation, APD can assist in obtaining the necessary assessments.

Provide evidence of Florida residency, such as a Florida Driver’s License or ID Card, Voter Registration Card, Declaration of Domicile, or other official documents indicating domicile in Florida.

Send the completed application and supporting documents to the APD office serving your area. Contact information for regional offices can be found on the APD website.

APD will review your application and documentation to determine eligibility. This process typically takes 45 days for children under 6 and 60 days for individuals aged 6 and over.
